By attending events of Mayflower Homeschoolers, I agree to the following Medical Release:

If I am not present at a Mayflower Homeschoolers activity in which an emergency occurs with my minor child(ren), and I cannot be contacted, I give permission for my child(ren) to receive emergency medical care. I authorize medical staff to provide treatment when a physician deems it necessary for the wellbeing of my child(ren).

Furthermore, out of charitable courtesy for the health of others, my family will refrain from attending Mayflower events if at least one member of my family is ill and until all members of my family have recovered.
